When responding to your request, we need to verify your identity in order to prevent leakage of personal information and spoofing.
Please note that we may ask you to present your photo ID (driver’s license, passport, or Basic Resident Register card) to match with the information we retain (only with visual confirmation and no copy required).

- Please mail us a document certifying that you are legitimate representative (such as “Letter of Authorization”) and a copy of identification document (driver’s license, passport, pension handbook, health insurance card, Family Register (extract), Basic Resident Register card, Resident’s card (within 6 months), or Alien Registration card) valid at the time of application.

In the case the person has died:
Certificate of All Records (Certificate of Family Register) with record of the person in question has deceased
Certificate of All Records (Certificate of Family Register) with a record that can be confirmed as the heir, or Will of the deceased that you are the executor of the Will.A copy of Agreement on Division of Inheritance.
For an adult guardian, a curator, or an assistant:
A document certifying that the person’s ability to act is limited (Family Register (extract), document certifying the qualification such as a family court certificate, a Certificate of Registered Matters) |
